2. Story Setup
The protagonist is a hospital patient named 川堀慎吾, admitted after suffering a head injury under unclear circumstances. He seems to have lost his memory and constantly rambles nonsense to the nurses and doctors. Even his parents won’t come visit him.
The one thing that’s crystal clear: this dude is an absolute creep.

Who besides a total creep would even say lines like that?
Thankfully, the hospital staff are surprisingly tolerant of him, especially the rookie nurse assigned to his room — 小夜啼 鈴芽. She’s kind, gentle, rocks a cute ponytail, and has that cheerful, energetic vibe everyone likes.

Then there’s the cool and handsome nurse 春鋤 筑紫, basically treated like the hospital’s resident prince. She’s admired by her coworkers, but behind that icy exterior is a surprisingly delicate personality. And under that tightly buttoned uniform? Yeah, she’s hiding a pretty impressive figure.

Next up is the beautiful trainee doctor 飯豊 風子, who always responds to you with cold indifference. She’s deeply passionate about medicine and usually buried in research, and she’s visibly fed up with your harassment — honestly just wants you discharged ASAP.

Finally, there’s the mysterious nurse who joins midway through the story: 羽咋 瑞保. With her striking pink hair and cryptic behavior, she gives off a suspicious vibe from the start, and for some reason, she seems oddly interested in you.

At first, these are just ordinary doctor-patient relationships. But through the protagonist’s warped delusions, these women are all one-sidedly imagined as his wives or concubines.
Convinced he’s their “master,” he decides to use his special talent — secretly filming them — to keep tabs on their every move inside the hospital.
As for what you’ll capture, and what you decide to do with that footage… that’s entirely up to you.
3. Main Objective
The core goal is to roam around the hospital, investigate different areas, and set up hidden cameras to gather information and footage.
By piecing together these little clues, you gradually uncover the heroines’ schedules as well as the shady undercurrents hiding beneath the hospital’s peaceful surface.
How you use that information is up to you. You can become a total scumbag who blackmails the girls with secret recordings, or you can act like the guy who saves them from the mess they’re trapped in. The game leaves that choice entirely in the player’s hands.

Will you blackmail the heroines and make them obey your every command?

Or will you save them from the shady people lurking around them?
4. Gameplay Loop
The game runs on a date-and-time system and heavily emphasizes replay-based resource management.
Each playthrough lasts only five in-game days, from Monday 4/14 to Friday 4/18. Every day starts at 4:00 PM and runs until 9:30 AM the next morning, for a total of 17.5 hours per day. Time advances in 30-minute chunks, which means 35 time slots per day, adding up to 175 total time slots across the full run.
During those 175 slots, every character besides the protagonist follows their own fixed routine. Your first task is to figure out each heroine’s schedule through observation and trial-and-error. Once you’ve mapped things out, you can use items and actions to collect key information or special items.
Meet certain conditions, and specific endings will trigger when you’re discharged from the hospital on the final day.

Each heroine has her own unique ending.
Because of the way the game is designed, there’s basically no chance you’ll clear everything in just a few runs. That’s why after finishing a playthrough, you can carry over items, money, discovered schedules, and other progress into New Game+ style loops.
So don’t stress about doing everything in one go — if a run feels like it’s gone as far as it can, just roll into the next cycle and keep building progress.
5. UI & Core Gameplay Breakdown
Earlier I mentioned that this game has a decent level of difficulty and is better suited for players who enjoy a challenge. That said, the difficulty here doesn’t come from tough controls or complex strategy. The real challenge is figuring out every character’s full schedule. Below is a quick breakdown of the basic flow and core mechanics.

Room 203 in the hospital somehow feels just like home.
Each morning, the player starts in their own hospital room, Room 203. From there, you have six actions available. Here’s a quick rundown:
- Shop on Amazon — Basically the in-game store. There are only five items available for purchase, which I’ll cover later.
- Monitor market trends — This option only unlocks after several playthroughs. It’s essentially investing, and serves as another way to make money.
- Call a nurse — Once your blackmail points reach a certain threshold, you can summon the heroines and threaten them. These points are earned by collecting hidden-camera footage.

The interface after successfully blackmailing someone — it says it consumes points, but it actually doesn’t. Translation bug.
- Online moderation — Basically a side job. It gives a small amount of money, but in practice it’s mainly used to skip ahead by a chosen amount of time. The cash payout is barely worth mentioning.
- Take a nap — Sounds like a short rest, but it actually skips an entire day. Handy when you’re speed-running resource farming.
- Go on patrol — Leave your room and wander around the hospital.
Next up is the interface while exploring outside your room.

The protagonist casually treats the hospital like his own backyard.
The upper-left corner shows the date, time, and your current money. The upper-right shows your current location on the map. The lower-right displays your inventory count. The full map screen also includes the heroines’ schedules and their personal info.
The hospital map in this game has three floors, with 50 rooms total, plus a rooftop water tank area, making 51 locations altogether. That’s one big reason why mapping every heroine’s schedule is such a pain.

Most of the hidden-camera spots are still concentrated around places like restrooms, bathrooms, and break rooms
Movement uses WASD, and the movement speed is pretty snappy, so it doesn’t feel sluggish. Walking itself does not consume time.
However:
- Entering a room or moving between floors costs 1 minute
- Triggering dialogue or performing actions costs 10 minutes
Since the heroines’ schedules operate on 30-minute intervals, you’ll need to pay close attention so you don’t accidentally miss a time slot.

Aside from Room 203, most rooms offer four to five actions. These are:
- Clean — Only appears in bathrooms and restrooms. This affects heroine behavior and becomes important later.
- Investigate room — Used to find key items or occasionally pick up money, though most of the time it just triggers flavor dialogue.
- Install surveillance device — Place a hidden camera in the room. There are three different camera types, each with slightly different uses.
- Retrieve installed device — Even if a camera records something, you need to manually collect it to review the footage. The top-right UI shows what type of camera is currently installed in that room and how much battery remains. If the camera successfully captured something, the footage will automatically be reviewed at the end of the day.
- Investigate other room — Basically just the “leave” option.
As for the H-scenes, they’re all presented as static CG images paired with dialogue.
Excluding the voyeur footage itself, each heroine has six scenario types, and each scenario has three story stages. Add in three NTR sequences, and that brings the total to 21 scenes per heroine.
That said, while each stage has different dialogue text, the CG changes are minimal — usually just slight variations. So visually, the actual differences are pretty limited.



At its core, the game is really just repeating the commands above over and over. There’s nothing especially complicated mechanically.
But like I’ve said several times already, the true hurdle is figuring out every character’s schedule.
Technically, you don’t need to uncover every heroine’s full route to clear the game. But if you want to collect blackmail material, trigger NTR events, or unlock the true ending, you absolutely need to be at the right place at the exact right time.

I only finished mapping one heroine’s schedule and already felt mentally drained.
So if you’re planning to clear this one blind without a guide, be prepared — this can easily turn into a serious time sink.
6. Tips & Walkthrough Advice
After making the game sound like a nightmare to clear, it’s only fair to share a few tips that can help make your playthrough a lot smoother and save you from wasting hours on dead ends.
Ending Requirements
At the moment, the three confirmed heroines you can fully route each have both a Happy End and an NTR End. The unlock conditions are hinted at in the gallery/recollection menu, but here’s a simplified breakdown.

As you can see, all of the heroines’ good endings follow a pretty similar formula: stop the NTR route at least once, progress their H-scenes far enough, grab a key item, and show up at the correct location at the correct time.

Each heroine has her own corresponding… uh… NTR partner.
One important thing to note is 羽咋 瑞保. Even though she appears in the heroine list and the gallery does list ending requirements for her, it’s still unclear whether her route is actually accessible in the current version.
I looked into some player discussions, and one theory is that you need save data from the studio’s previous game 満車率300% 3≒ to unlock her route. That honestly wouldn’t be surprising, since this dev team has used that kind of cross-save gimmick before. Unfortunately I didn’t have the previous title on hand to test it myself, so if anyone manages to confirm it, definitely share the info.
Replay Loop Mechanics
As mentioned earlier, the game is clearly designed around multiple runs. You’re expected to replay it a lot, so there’s no need to force everything into one playthrough.
When starting a new loop, the following carry over:
- Items
- Money
- Blackmail points for each heroine
This is huge. While some items are consumable, cameras themselves can be reused, and since gathering hidden footage efficiently is the name of the game, having more cameras makes life way easier.
Money earned from in-game part-time work is painfully slow, so the better strategy is abusing the loop mechanic: if your carried-over funds are below 50,000 yen, the game automatically tops you back up to 50,000.
That means before ending a run, spend all your cash on cameras and supplies. Then start the next loop with a fresh 50k again. This makes stockpiling gear way faster and saves a lot of frustration when you’re doing a serious clear attempt.

Trying to buy a full set of equipment through the in-game side job takes forever.
Item Overview
There are only five items in the game:
- Outlet Camera
- Pen Camera
- Battery Camera
- Diuretic
- Aphrodisiac
Quick rundown:
- Outlet Camera — Unlimited recording time, cannot be removed, audio only
- Pen Camera — 3-hour recording time, cannot be removed, records video + audio
- Battery Camera — 8-hour recording time, can be removed by NPCs, records video + audio
- Diuretic — Can be slipped into a heroine’s food to alter her schedule
- Aphrodisiac — Same idea, changes heroine behavior
My guess is the developers intended players to first spam cheap outlet cameras everywhere, then use the audio logs to map out schedules, and finally switch to the expensive video cameras for important scenes.
On paper, that sounds pretty clever.
In practice, once you realize there are 51 locations and cameras are expensive as hell, it becomes way less fun than it sounds.

To get the better CG scenes, the camera has to catch the heroine during the exact time she’s taking a bath.
Personally, I’d recommend the Pen Camera over the battery version. Sure, 8 hours of recording is tempting, but if the battery camera gets removed, that event is gone until the next week cycle. Unless you’ve already farmed a mountain of cash, the pen camera is just more reliable.
Drugging Mechanics
The other two items are the diuretic and aphrodisiac, which let you alter heroine schedules by drugging their meals.
Important detail: you can only do this during a specific window — 5:30 PM to 7:30 PM in the second-floor cafeteria, and only when none of the heroines are currently there.

Three of the heroines are mainly on the second floor, while the other one is on the third floor.
You can choose how many pills to use, from one to five.
Based on the recollection gallery:
- Diuretics have both weak and strong states, which count as separate hidden-camera materials
- Aphrodisiacs don’t seem to have separate levels; likely just success/failure
Hidden Footage & Blackmail Points
These two terms come up constantly, so here’s the simple version.
Blackmail points are the currency used to coerce heroines into sexual events.
You gain them by collecting compromising hidden footage of each heroine.
Each heroine has her own separate blackmail point meter — they are not shared.
A key detail: recording the exact same footage multiple times does not give extra points.
At the same time, the game splits footage categories into absurdly specific variations.
For example, secretly filming a heroine using the bathroom counts as four separate footage types:
- Normal
- Bathroom (Aphrodisiac)
- Bathroom (Weak Diuretic)
- Bathroom (Strong Diuretic)
Each of those counts separately toward collection.
If you want to track what you’re missing, the recollection gallery is the easiest reference.

You can tell just from the gallery how ridiculously specific the footage categories are.
Standard Voyeur Scenes & Encounter Voyeur Scenes
Besides regular hidden-camera captures, there’s another category called Encounter Voyeur.
These are even more annoying to unlock.
They are not obtained by placing cameras. Instead, if you enter a specific place at a specific time while a heroine is there, a short event scene and CG automatically trigger.
Each heroine has 9 of these.
And yes, realistically, the only way to complete them blind is brute-forcing almost every time slot and location combination.
If you’re a completionist, good luck — you’ll need it.


Cleaning Option
Only bathrooms and restrooms have the Clean option.
Its purpose is simple: it makes heroines willing to use those locations for bathing or bathroom events, giving you a chance to record them.
Drugging can also force some changes, but during certain time slots, if you haven’t cleaned those rooms, the heroines simply won’t show up there at all.
So if you have spare time, just clean them daily. It’s low effort and almost always worth doing.
Night Patrol
While player movement is mostly unrestricted, there’s one catch: from 10:00 PM to 6:00 AM, nurses patrol the hallways.
If you’re walking around outside your room during those hours, there’s a chance you’ll get caught.
If that happens:
- You’re forcibly sent back to Room 203
- One extra hour passes automatically
There’s no major punishment beyond that.
However, there is one exception: if the head nurse catches you too many times, it can trigger a special Bad End.

The head nurse seems like she’s probably a DLC character.
7. Final Thoughts
ナースコール警備員 is the second title from ベルゼブブ that I’ve personally played. The previous one was 満車率300% 貳 全線開通版, and I have to say the overall style feels very similar, especially when it comes to how heavily the game leans into time-management mechanics.
I honestly did not expect anyone to design a schedule system this convoluted and then ask players to brute-force their way through it just to figure out how everything works. That said, for some players, that kind of trial-and-error gameplay might actually be exactly what they’re looking for.

Looking at a timetable like this, does it feel strangely familiar?
Gameplay itself is fairly straightforward. You repeatedly place cameras, retrieve them, and use the collected footage to unlock the conditions needed to progress.
The H-scenes are presented as static CGs paired with text and voice acting. There are definitely quite a few reused variations, but overall the CG quality stays consistently solid. On the technical side, the game runs smoothly too — I didn’t run into noticeable lag or any major bugs during gameplay.

The CG artwork stays consistently solid throughout.
That said, the current Chinese translation is genuinely rough.
It’s not just small typo issues. Character names randomly change from scene to scene, kanji errors show up all over the place, and sometimes the text straight-up switches into romanized Japanese. Dialogue terms and grammar are all over the place, and the H-scene writing often becomes nearly incomprehensible — like a world where nobody actually speaks Chinese, but everyone is somehow forcing themselves to communicate in it anyway.
Even worse, some translated variables seem to break entirely, which causes important location names or event images to fail to display correctly.
I also tested the Japanese version myself, and everything worked perfectly there, so this is clearly just a localization issue rather than a bug with the game itself.

The Japanese version correctly shows where cameras are installed — the Chinese version, though… yeah.
Even with those problems, I’d say the game still carries the studio’s signature style and quality.
That constant pressure of racing against the clock, the thrill of collecting private footage through hidden cameras, and the blackmail/NTR event chains are all here and very much intact.
While there are currently only 3+1 heroines available, if you’ve followed this developer before, there’s a pretty good chance they’ll release additional character DLCs later on — that’s been their usual pattern.
Still, if you don’t understand Japanese, I’d strongly recommend checking whether the translation has been patched before buying it.
